If you have the immigration issue, please fill in and sign this CIC consent form (boxes 1,2,3,4 and 7).
To find out about someone else’s immigration file (e.g. a relative) that person must fill out and sign the CIC consent form (boxes 1,2,3,4 and 7).
If you are enquiring about someone else’s immigration file you and that person must both fill out and sign the General consent form, otherwise the office cannot communicate with you about the file.
In order to sign the consent forms, you need to be 18 years or older, otherwise you need an adult to consent on your behalf.
Once completed, all forms may be sent via email to (firstname.lastname@example.org) or you can hand deliver them to our office during business hours.
